I think it's worth just mentioning Simon that I do see a fair frequency of self presenters with DKA. It is definitely worth highlighting the fact that they might not have had any pre-hospital treatment at this point. If your patient does have hypotension so if that blood pressure is low they do need that fluid bolus initially. But basically the bottom line is that we need to stop the ketone production but that glucose and fluid normalization needs to happen in a controlled and not overly aggressive fashion.
