There was a bug I dealt with in my previous role where we knew the bug existed for two years. We could never figure out the set of steps that users were going through to hit that flow. And finally our product manager just happened to be poking around one day and found the right combo that triggered it. The way that you get to those combinations is just incredibly arcane.
