We are becoming more open and honest to a becoming less of a blame culture. We are addressing errors differently, more as a systematic approach rather than an individualized approach. And i would say that if you are a registrar, or any training grade doctor, the reason why thet er th there is always a consultan on call is for exact situations like this. So you should always feel in power to get a second opinion in a situation like this. I'm talking human factors here - what clinical biases maybe did you have?
