Purosemide is probably acting via at least four different ways in acute congestive heart failure. The main way it reduces dyspnea is rapidly decreasing the left ventricular filling pressures. It does so via venodilation, upregulation of prostaglandins and in the endothelium. And then 30 minutes later, they'll die-erase.
