I find it very difficult to understand how an antibody to an intracellular target can be or certainly directly pathogenic. Most people would then say that the T cells mediate that disease. What we're seeing is clearly antigen specific. So there's a dominant antigen which has reached immune tolerance in some way. And then it would make sense if some of these conditions were very difficult to reverse, because there's been cell death.
