The Importance of Decompression in Traumatic Pneumothoracies

The majority of our interventions and treatments should be undertaken en route to the major trauma center. The classically taught area for insertion is the second intercostal space in the mid-clavicular line, just over the top of the third rib. But we know from multiple studies that this can lead to failure of decompression, particularly when an IV cannula is used. So I guess there's two ways to overcome that issue, either you use a longer needle or you change your decompression site.
