After action review, i call it a post mortem. And i don't like the term because post mortem implies that there has to be a dead body. Let's at it off a failure. It was taken from from medicine, from autopsies, and then implanted into the business context. After action review is a much better way of putting it - you're just reviewing the action regardless of whether it produced success or failure. The same idea applies to failure too as well. A lot of business leaders will try to change everything when a failure happens. But that's not necessarily the right course of action. Sometimes good decisions lead to failure.
