Nephrologist - Is the Patient on CVVH?

If a patient with sepsis is on intermittent HD or in renal failure and we haven't started HD yet and they move to CVVH, a really important point to consider is antibiotic and medication dosing. Prevention is the key and we have great medications now that can reduce the rate of kidney decline. Patients should be on ACE or ARB inhibition and managing blood pressure, managing diabetes. Our goal really should be that no patient needs to go on dialysis that we can prevent kidney failure and progression.
