An Investigation of a Separation of Engine Number Three
The board concluded that the separation of the number three engine pylon was caused by a failure of connecting components. The board thought of nine scenarios that would lead to this happening. Of the nine, six of them were quickly dismissed, leaving three possibilities. It is assumed that the inboard sheer face of the fuse pin was initially fatigued and then failed under normal flight conditions.
