Is the Pacing Threshold in a Defibrillator a Good Idea?

Most patients require a current somewhere in the range of 20 to 140 milliamps if you look at some of the literature. So it really is a case of just starting low and working your way up until you gain capture. But we do know that thresholds are usually elevated following thoracic surgery in patients with emphysema or pericardial effusion.
