How Common Are Clinical Pharmacists in Hospitals?

In most arcademic medical centres, there is going to be a clinical pharmacis that touches each team. So i may interact as a nice u provider with multiple pharmaciss because i'm taking care of patients on multiple different services. And then on the back and i'm thinking, well, this patient, we change their blood thinner. I need to let their primary care physician know i need to call and cancel this prescription. And their pharmacy. They might just have a pharmacis thits in your eye to you a,. or you might have one that's in the central pharmacy that also kind of looks over patients if there's a question that comes up.
