The Role of Ultrasound in Diagnosing Torsion
The Whirlpool sign is a twisted sromatic cord with a spiral pattern. If you find this on ultrasound, it's diagnostic for torsion. A pseudo mass is made up of a congested epidinumus, vast deference, and vascular bundle right below the point that theSromatic cord is twisted. The two other ultrasound finings are a horizontally lying tesicle and tesicular enlargement from vascular congestion. These are all highly suggestive and they don't necessarily rely on Doppler flow. This seems like a lot with ultrasound but the big takeaway is that you should not use a normal Doppler to rule out torsion."
