There's no perfect study design. If we had unlimited resources and we could control people totally, then we might answer each question we have with a large randomized trial. That would be many thousands of people go on for many decades. But that's virtually never going to happen. An advantage of the type of what has been called an observational study that we do is that first of all you can look at the long-term effects of dietary factors.
