The Importance of Sustained Breathing Trials

When we're talking about extubation, I hope that they're usually on an FIO2 of less than 40. That is kind of my general rule of thumb. At our institution, we utilize a standard level of pressure support based on the size of the endotracheal tube. So if you are going to ween off or extubate on a peep of five, you want the peep to be five and then you add that pressure support on top of it. But there are lots of studies that support the idea that actually pressure support is over supporting the patient.
