Metabolic acidosis with a bicarp of one has actualy a pretty small differential. If you've got the really high serum osmalality, zero ethanal and a metabolic acidosis, i would almost say like, boom, you may have your diagnosis. There's a bunch of other clues in the lab work, but they're a bit less specific. You can actually get a hilactate, which might mislead you into thinking that your acedosis is just from some other cause of hilactates.
