estrogen is pro-cognitive, that there are estrogen and sex hormone receptors in the prefrontal cortex through which many of the networks travel. The ADHD woman going into perimenopause will complain cognitive ability is worse. You have to decide whether ADHD predated the chemo brain. It's a trajectory, not a cross-sectional symptom checklist. And so it is very hard to come up with a plan to treat this for them. If you're seeing a different clinician every time you go on HRT or do you stop your tamoxifen? Do you increase your stimulant medication?"
