When you're n ipe fellow the cases just fall in your lap. You have to think like a polmonologist first, before you start instrumenting the patient. It's really who needs a bronk? Who needs a trake? What type of patients are we getting by putting them through this risk? The decision-making is truly the hardest thing about becoming an attending rule.
