The most important thing is to try to get as many of those records as you can. It looks like he was scheduled for an ambulatory surgery center and then was moved over to the main hospital. He likely with moderate to severe pulmonary hypertension probably needed to be moved. I'm also concerned that maybe he has valvular disease, so I would want to get that information as well.
