Transmissability becomes the more significant factor. We have so many more people getting infected that even though there's a smaller percentage over all of severe cases, that absolute number is so large it does still overwhelm the health care system. And i'm guessing the way around scenario too, which sounds much in scenaria one, is making sure people get boosted and take safety precautions.
