Chronic hyponatremia is associated with osteoporosis and bone loss. It's worth trying to improve because it does increase their risk of having complications the lower it goes. If you can get it up by better managing their disease, then you should do that. I wouldn't go give it a hypertonic because now you're giving it a big sodium load to somebody that doesn't need sodium.
