The Risks and Benefits of Delaying Therapy

In my opinion we typically never delay starting therapy because we have active interventions that can be used to manage the side effects. There is always the theoretical risk of ectopic calcification from hyperkylecemia and hyperphosphatemia clinically it's not something we worry about as much. We see these patients very frequently in the pediatric intensive care unit and at times they are unable to be medically managed. How do you approach CRRT in patients with tumor lysis syndrome? I really think that it's such a case-by-case basis as well as balancing the risks and the benefits.
