Heparin Drip and Post-Op Bridging
The guidelines recommend only doing is for patients with moderate or high risk of stroke or thrombotic event. You can start a heparin drip without a bolus, so thought to be a little less risk of bleeding but still some protection. The highest risk for a post-op bleed when you're bridging is when the INR gets therapeutic.
