A lot of people die because they couldn't afford the medication. Insurance in my experience has never said no to a medicine. It doesn't make sense for them politically or financially. The drug business is $500 billion business. They don't pay attention to small drugs like their and say, when this drug becomes a billion dollar drug, then we'll think about whether or not we can limit its use.
