The quality of evidence was very low for each recommendation, although the reason that it was low varied from recommendation to recommendation. I was surprised to find studies that inform many of the recommendations in the guideline. We're quite old, decades old, and many of the things that we have learned and do automatically in clinical practice really are not informed by very much empirical data at all.
