How to Work Up Thyroidism

In patients who you're suspecting might have Graves' disease, start with a TSI and TBII. And for those patients, the scan actually has low uptake because the thyroid is not pulling in new iodine to make new hormone. It's just dumping out what it already has. Another one would be if you actually felt a nodule on exam or perhaps obtained an ultrasound that showed some nodules,. Then again, you might be concerned that one of those nodules is overactive. The radioiodine uptake scan is helpful because it could show if one area is taking up a lot of iodine.
