There was really only a couple of patient ham episodes out of all the incidents, and many were what i'd call more administrative. We have a process where the access nurse or team leader does safety bay cheks and checks all running infusions a twice each day. And the large majority of these incidents were near messes,. things like missing a missing second signature, or an infusion that had la overran its expire time, usually by a bit.
