How to Deal With Sarcoidosis

Is there a standard form or questionnaire that you have for the patients with sarcoid? Is it, or is there something available publicly that our audience can use if they have a patient they're suspecting? I mean, there are articles that can go through your workup and your history and sarcoid. I have a templated note that I use because I just can't remember. And I actually ask those questions every visit really.
