How Did the Feedback Forms Work?

In the early lectures, i was trying to get students to think about tunnel visionit is a probmie sene over and over again with high achievers. I've watched a lot of my student won'tn't go through it since then. The feedback form, i'd love to talk about the feedback form. Were the feedback forms standardized, so you didn't have input into the feedback forms? Or were you able to design questions that you then gave to these sort of early student slash victims of your many, many rehearsals and trial run? In either case, i'd be curious to know what you did, what you paid attention to in the feedback forms.
