The kidneys must excrete 50 to 100 mill equivalents of non-carbonic acid generated every day. This is achieved by hydrogen secretion, although the major mechanisms are different in the proximal tubule and thick ascending limb of Henley. The daily acid load cannot be excreted as free hydrogen ions since the free hydrogen ion concentration in the urine is extremely low. We need to smuggle the hydrogen out as some other compound.
