How to Dose Cannabis Properly

There's a bell-shaped curve when it comes to dosing of cannabinoids. Doses that are too low or doses that are too high tend to be non-therapeutic. I start patients out very slow, between one to five milligrams per dose starting out with THC. And then we can always ramp it up, you know, from there.

I love that. That's very helpful because that does, that is a small set of the population that's on those things. So, like you said, for those of you who are on one of those, get somebody to help you monitor them, but for most everybody else, the rest of your medications are probably, like you said, if you have a little bit too much, a little bit too little omepersal for your acid reflux, not as big a deal as the others. Yeah, let's talk a little bit about how you advise people on dosage. Uh-huh.

So, you know, there's, the thing about that's really unique and I think very beneficial about cannabinoids is the fact that there's a bell-shaped curve when it comes to dosing, meaning that doses that are too low or doses that are too high tend to be non-therapeutic and there's a sweet spot, you know, which is another thing that sets it apart from opiates, for example, where people just find that they have to keep escalating their regimen where cannabis is very different than that. Cannabis and, you know, a lot of people that say, well, you know, I was getting really good results, so I started to increase the dose of it and then all of a sudden I started to not receive the benefits of it. And we know that that's been shown over and over again. So I'm a big proponent of the start low and go slow, you know, idea. And you know, so when we do that, you know, I start patients out very slow. I mean, I'm talking anywhere from one to five milligrams of THC to start, you know, for the cannabis-naive individual where, you know, those who are not cannabis-naive, which, you know, is really realistically even in, you know, even in New York State where it's, you know, still illegal, I have several patients come into me that say, you know, I smoke regularly from where I get it from my friends or whoever they're getting it from. You know, and a lot of times they'll come in and tell me, I take these gummy bears that I got from out in Colorado that are 10 milligrams apiece and they work great for me. So that's extremely beneficial because I know already that they're, you know, how they're going to respond to it. But for individuals who are naive to it, and particularly the elderly, when it comes to THC and wanting to avoid the risk of falls and confusion and any sort of mental status changes, we start very low. You know, I start very low, like I said, between one to five milligrams per dose starting out with THC. And then we can always ramp it up, you know, from there. And some of the side effects that people get from THC, we also see that if you start low and go slow and slowly increase it, those side effects tend to go away, or at least the perceived side effects go away, meaning they get used to it. So the dizziness, the dry mouth, the fatigue, a lot of these things that people will initially experience, we know, tends to improve over time through continued use.
