High risk patients that don't have any contraindications can also be considered for an allogenetic stem cell transplant upfront. For hypoplastic MDS you can treat those patients like the treatment for a plastic anemia which is atg with cyclosporine. And then finally another pearl is for patients with a history of many transfusions so if patients have a history of 20 to 30 blood transfusions they may have iron overload.
