If you have advanced atherosclerosis characterized by successive ruptures of the plaque and rehealing with scar tissue and clots that build up, you may get to a point where there are certain sites that aren't totally reversible. Even in an advanced plaque where you have successive rupture and rehealing and repeated instances of clotting, it's still the buildup of oxidized lipids in the area promoting inflammation. Stable plaques are not the cause of ischemic strokes or myocardial infarction. The only way that you can get a cardiovascular event from stable plaques is if they are so in overwhelmingly enormous and obstructive in all three coronary arteries.
