If this is something you want to share with an admissions committee, i think you could include it in a diversity statement if it doesn't feel like it fits in your personal statement. An identum is meant to be very short, very factual, no drama, no narrative. To the extent that your neuro diversity or your history of mental health issues is cord t or identity, you can certainly feel free to discuss it in that aspect of your identity or background. No details are required. And in fact, i think better to have very little detail.
