Remifetinol in Renal Failure

Fentanyl, fentanyl and remifetinol don't build up to have any significantly prolonged effect in renal failure because they are still metabolized by the liver into inactive metabolites. Hepatic failure really has a relatively minimal effect except that it can reduce hepatic blood flow. The effective age on the pharmacokinetics of opiates is significant. Older patients should be reduced by at least 50% if not more in older patients.
