The NTSB's Investigation of the DC 9 Accident
The NTSB believes that this role reversal contributed significantly to the eventual runway incursion. The investigation revealed several areas of faded or nearly invisible taxi lines, especially near the area where the DC 9 was taxing. Another confusing factor was the Oscar six sign located between the inner and outer taxiways. If they had continued instead of making this turn, they would have been okay.
