Flight Crew Failure to Diagnose Pressurization Problem
Findings and the causes from the report right here. The flight crew probably lost useful consciousness as a result of hypothesea some time after their last radir communication on the company frequency at six 20, approximately 13 minutes after take off. Inadequate c r m contributed to the failure to diagnose the pressurization problem. Incapacitation of the flight crew due to hypoia, resulting in continuation of the flight via the flight management computer and the auto pilot. Depletion of the fuel and engine flame out and impact of the aircraft with ground. Soere three different check lists where they missed it, pre flight, before start and after takeoff.
