High HDL does... Is a marker of lower risk and lower HDL is a marker of higher risk. It can be used to predict risk in a model, either individually or in a population. But it's not clear that changing up HDL levels in either direction has an impact on risk. And then there are drugs that raise HDL. Those drugs have been tested again and again and again, and it doesn't appear they do anything to impact risk. So cholesterol, and specifically the able lipoprotein B containing lipoproteins are still the bad actors.
