In more than 90% of cases with vomiting of blood or coffee grounds, there's going to be a lesion that's proximal to the ligament of trites at the junction between the third and fourth portion of the duodenum. Non-strotal anti-inflammatories are a leading cause of GI blood loss. Seeing fresh red blood from below, again, not a guarantee that this is a colonic source.
