The Pilot's Approach to the First Officer
The NTSB concluded that the actions of the captain led to a steep approach and distractions of the first officer from his primary tasks. The safety board said Erickson was distracted from his duties as a result of untimely and poorly planned instructions from the captain. As a result, the pilots experienced a loss of altitude awareness and failed to correct the situation before ground contact was made. It also criticized the airline for not dealing with the captain's conduct saying that company management did not address these matters adequately.
