What the Hell Is That on Your Back?

Garry went to the local doctor who said they'd never seen anything like it. The holes in his back were, like, all the skin was cut out inside the circle. So my logical explanation is that something bit me and took the skin off my back. But how that creature got underneath the rash gart and then didnt leave any holes in the rash guard?
