Dr. Sears: Co-arctation of the aorta can be severe, but usually well tolerated in utero because about two-thirds of cardiac output flows through the PDA into the descending thoracic aorta. After birth, however, the increasing amounts of cardiac output must bypass this constriction. The mechanisms at the heart can use to compensate for this extra afterload are one, hypertrophy, and two, increasing the LV dimension.
