The Flight Engineer's Failure to Close the Blee Air Valve
Speculation is that the engineer was goig throughis check list, trying tofr ut hat's going on. The captain allowed himself to remain removed from the control loop by leaving the auto pilot engaged. As a result, he was not ware of the increasing control inputs required to maintain level flight. It's just like everything goes crazy causy.
