Ferritin and Transferrin Saturation in Pregnancy

By the third trimester, there is no credible expectation that the eight milligrams of absorbed iron per day that's necessary to meet the placental and fetal needs will be achieved by taking oral iron. For a pregnant woman, I think a ferritin less than 30 is a pretty good indicator of a need for iron repletion. The new KEDGO guidelines are our T-Sats, less than 40 with ferritins, less than 500. Iron restricted erythropoisis? That's somebody with liver metastases. You can just ignore the ferritin in that situation.
