The FDA is trying to use real world evidence in place of randomized control trials. But when you're talking about mega trials with over 40,000 people, the benefit risk calculus doesn't make sense there. The idea that patients are being taken on by the person that receives a vaccine just won't be occurring and that's troubling stuff.
