Naltrexone is contraindicated for people who have a co-occurring opioid use disorder and alcohol use disorder. The best thing to try, especially if the reason that they're using is because they're in pain, is buprenorphine. It works for pain, it treats both, and it treats their opioid use disorder. And if their pain is well controlled, then maybe they decrease or stop drinking.
