A 79 year old man was brought to the emergency department by his daughter for five hours of confusion. He has a white count of 30,000, a creatinine of 2.4, baseline is 0.8, and the UA with pyuria, bacteria. The urine nitrite is negative; there's more than 100 white blood cells, more than 100 red blood cells. His EKG and chest x-ray are normal. And again, not really sure why he's in the hospital.
