When students are struggling it's really important to scaffold the classroom so as not to put them on the spot for reading out loud or reading in groups. I think reducing the amount of independent reading time or partner reading time for kids who are really struggling is huge particularly in the structure of reading workshop models. In terms of writing assignments when kids are really struggling with language using word banks use sentence stems to get them started.
