Is the Incongruent Affect a Mental Disorder?
In terms of irritability it can be both as an affect which you're seeing and it can also be the mood that the patient is describing. So when I'm in a situation where I'm not exactly sure how to state what's one word, I describe it because then the reader knows exactly what is going on. And you can also put subjective objective or presentation. In the affect is of course it's an objective assessment of what was going on. In mood you can say objectively the patient is reporting all depressive symptoms but you know subjectively she's saying she's not depressed. You could add that as a qualify.
