Big companies are able to aggregate more information and get more insight into the process. And it does disadvantage small companies that don't have the same resources who might just have a single product. Development costs have gone up enormously. Enrolling a single patient in clinical trial of a cancer drug could be literally a couple of hundred thousand dollars, just a single patient.
