Not doing the requirement stages is a pretty big one. There's an unstated undercurrent here that says, we have to get started writing code right away because it's going to take us so long to fix it later. I think if most organizations took a look at measuring how much of our total effort is spent doing things over that maybe we didn't have to do,. sometimes you might find that you could get a third of your bandwit back by taking a different approach. And i'd probably find that we come out ahead that way most of the time.
